Program/Policy Development Coordinator Glenn Baly was not getting the desired health outcomes from his population. In plan year 2015 2016 only 22 people signed up for his in-person diabetes prevention program and only 9 completed the program out of an estimated 40,000 eligible members.
Canary Health deployed Virtual Lifestyle Management, our year-long digital diabetes prevention program as well as our six-week peer-led, small-group online workshop Better Choices, Better Health to OEBB’S population. Intervention began with outreach to eligible participants to engage them in self-managing their health, and keep them engaged throughout the programs.
•Both programs generated an 80% completion rate.
•For VLM, registrations went from 22 to more than 400.
